CBT receives the status of a golden child of therapy. People discuss it as though it is the only miraculous thing that will work in everything, Anxiety sorted, Depression sorted, Overthinking sorted, OCD sorted, a little stress sorted. And CBT, it is wonderful and structured and everything. But be honest with me one moment, It doesn’t always work.
And when it fails, the majority of the individuals get panicky. They believe that something is amiss with them. They believe that perhaps they are hopeless, or perhaps they are too complicated a case, or worse still they suppose that therapy in general is futile.
Here’s the thing. You should never think that CBT fails unless it is nearly always your fault. It is normally about fit, timing, structure, complexity or even therapist-client fit and there is even a science of why CBT does not make the mark in certain cases.
So First, What Is the Real Failure Rate of CBT?
One of the largest meta analyses reveals that not more than about 40-57% of individuals with depression show any improvement with the help of CBT. This is not bad, but not very good either.
The other one examining anxiety disorders found that in the range of 49 to 54 percent it works out well, and with such conditions as OCD the response rate reduces to 38 percent which to be quite frank is really not very high considering all the hype surrounding CBT.
Another interesting thing. Relapses actually exist even where CBT is effective.
Certain studies demonstrate that, in 1 year, there is approximately 32% relapse of depression. In two years, the relapse may increase to 54 percent.
So the myth that CBT solves everything permanently isn’t true. Reality is much more complex than that.
reasons CBT fails to work with everyone
The Issue is Too Complex For Basic CBT
CBT is marvelous in easy clean problems.
However, when a person is dealing with several things at once such as depression and trauma and relationship stress and chronic illness then a simple CBT guideline is usually too superficial. Complex cases require complex CBT or extended CBT that is a combination of more elements and sometimes more time.
The Therapist/Client Relationship is Not Hitting
This is honestly underrated. The therapy method may be the best in the world but the vibe will not land.
CBT particularly requires trust since there is homework, exposure activities, and thought work. CBT becomes mechanical in case a client is not free to open up.
Homework Not Being Done
This is what people do not discuss.
CBT relies much on homework. But life happens. People get busy. People forget. Worksheets are not something people can associate themselves with. People lose motivation.
Once the homework falls behind, the progress slows down. It’s not really the client’s fault. It’s the structure.
Culture And Context Matter More Than CBT Textbooks Acknowledges
Many models of CBT have been developed in Western cultures that are individualistic.
But here in India:
- family involvement
- collective expectations
- financial pressure
- societal norms
Each of the above influences mental health differently.
When CBT is being taught out of context it is alien. Sometimes even insensitive.
Misdiagnosis or Underdiagnosis
Sometimes it is not what it appears to be, or perhaps one believes he/she is handling a case of anxiety but there is unresolved trauma, or one goes to see depression but he or she has bipolar depression that is to be treated differently.
CBT cannot or will not FIX SOMETHING which is not even identified properly.
Timing Terrible Timing
In case an individual is experiencing a crisis.
- exams
- breakups
- job loss
- family conflict
- health issues
then CBT might seem excessive toil.
The brain is overloaded to the extent of restructuring thoughts.
HERE’S THE GOOD NEWS
CBT failing does not make it to say nothing will work.
It just means we adjust. That is literally what the modern therapy is personalized, flexible, client centered. Not stuck to rigid formulas.
Now let's discuss the strategies that can beuseful when CBT isn't working.
Replacement of Basic CBT with Enhanced or Complex CBT.
Basic CBT is short-structured.
But enhanced CBT can include:
- emotion regulation
- behavioral activation
- schema work
- mindfulness elements
- values based work
- cognitive restructuring.
This is particularly useful with those clients who have patterns that are long lasting, childhood conditioning or a history of trauma.
As a matter of fact research indicates that elaborate CBT regimen yield more lasting benefits than simple CBT in most instances.
Include CBT Into Other Therapies
Frankly speaking this is where magic does occur. Pure CBT is not generally good when compared to a blended therapy approach.
Some effective combos:
- CBT plus ACT
- CBT plus DBT skills
- CBT plus mindfulness
- CBT plus exposure therapy
- CBT and medication of the severe cases.
Adjust The Pace Don’t Rush
In case CBT failed it may only imply that the individual was not willing to plunge into the area of cognitive restructuring.Perhaps they should have been stabilized. Perhaps they required skills on how to ground. Perhaps they had first to be emotionally literate.
A slowing down is usually a lifesaver of the entire therapeutic process.
Use An Alternative Format of Delivery
Other individuals are not fond of face to face therapy, some prefer online, some prefer group settings, others favour formal courses.
It is in this case that cbt course online can be a game changer particularly to young adults or even working professionals who prefer to work at their own pace.
CBT is made easier to digest occasionally when the mode is shifted.
Explore Biological, Social and Environmental Factors
Since nothing is magic in therapy, If someone is:
- sleep deprived
- eating poorly
- working 15 hours
- stressed at home
- residing under toxic conditions.
- addressing hormonal changes.
CBT will not solve everything on its own.
Therapy is most effective when the body and the environment are not in opposition to it.
Strengthen The Therapeutic Relationship
In other cases, it is the lack of safety. In some cases, clients are reluctant to discuss discomfort with the therapists. In a few cases, therapists suppose that the client is okay with the structure. Even minor change in communication will make a significant difference.
Make Psychoeducation More Straightforward
Many CBT fail to work since clients are not well informed about the reasoning behind it.
They believe that it is simply worksheets, but after knowing how the thoughts cause cycles and how the behaviors change with the knowledge that the work is meaningful, then that is when the work becomes meaningful.
Admit That One Does Not Aspire to Perfection
CBT is not the one that eradicates thoughts.
It is regarding how to alter responses. When individuals believe that therapy would help them forget about anxiety or sadness they are disappointed. A renegotiated objective reduces the pressure and enhances reaction.
what should the clients do when CBT is not working for them?
Consider What Role Was Not Effective
- the homework
- the approach
- the therapist
- the timing
- the emotional readiness
Request A Therapy Follow-up Session
therapists tend to redefine and change strategies.
Examine Other Modalities
- ACT
- DBT
- somatic work
- mindfulness
- psychodynamic therapy
Combine CBT With Lifestyle Modification
- sleep
- movement
- nutrition
- boundaries
- stress reduction
Participate in Group Programs/ Online Courses
Learning among others is sometimes more motivating. Attend any cbt course online with therapists. CBT is less overwhelming with self paced modules.
Final Thoughts
CBT Not Working Is Not The End.
It’s Literally Just Step One
Anyway here’s the thing. Therapy is not supposed to be stagnant. Do not push you into a fixed pattern which is unnatural. In case CBT failed there are a hundred ways to alter it. The tiniest changes have the greatest discoveries sometimes. You’re not failing therapy. Treatment only should be modified and honestly that’s normal.
